This is to certify that I, parent/guardian of named child/children, assume full responsibility for their actions during Nature Bugs play session at Potager Garden, Constantine.

I agree to release Nature Bugs and hold them harmless from any liability which may arise from incidents or accidents involving my child/children and myself whilst participating in the Nature Bugs play session, to the fullest extent permitted by law.

I understand and have been made aware of the presence of plants in the greenhouse which could be harmful if touched (cacti collection and giant aloe) I take full responsibility for keeping my child/children and myself away from these plants and release Nature Bugs from any and all liability.

I have read and understood the above liability waiver form and by ticking this box, sign it freely and voluntarily without inducement.
